"How are you doing" Is a call-and-response looking for a "yeah, how you doin'?"
"How are you feeling" is an inquiry about your health.
I sometimes ask if they want to know for real or just being polite.
It's about 50/50 it seems.
When someone asks how you are doing just answer "fine".
Unless they ask like Joey from Friends. If they do that, assess your options.
