Because first and foremost, these people are authoritarians. Like Jews for Hitler, their craving for the taste of a boot, and to see those they regard as 'degenerates' as smashed under it, is far greater than their own sense of self-preservation.
I think there's also a contingent that is anti US no matter what, so anyone against the US is good and whether they're a dictator is a secondary concern.

You can still use the lever clearly, simply flip the lever after one axle of the train has passed over the switch, but before the other axle has. This will force a derailment.
What happens to the people on the trolley? Or those nearby?
