Every time I see one of these charts and it shows some team being bad at either defense or offense, somehow the next game we have with them they turn into the greatest team in the league.

I think the two surprises are to the right of the chart: Celtics and Knicks. Top 2 offenses, but their defense has declined (cough cough KAT) which explains why Celtics have not looked great, and why Knicks are where they are.
