It was powerful but non-specific. That's an anxiety everyone can have, so while the intended target just immediately got destroyed, everyone else has to make a save to avoid becoming affected themselves.

My guess was it was so effective that everybody in earshot of the insult is getting blasted.

That's how I took it. It's so powerful it insta-killls, then by default everyone within earshot thinks about what was said and needs to save else they also take the depressive damage
